flash flash contact form php actionscript action script customer contact form help deskThis is a very simple contact form that can be used on any website.  It's sleek and very professional looking.  It uses an exterior PHP file to process the input and send the email message to the recipient.  It does include validation that checks to make sure that all fields contain information before the email message is sent.  You can view a demo of how this form looks and operates by visiting our contact page.

:: Server Requirements
This script will work on any Linux or Windows server running PHP 4.3+.

:: Cost
The price for this add-on Flash module is $15.  We'll even change the background color of the form before emailing to you.  Just let us know what color to change it to in hexadecimal format and we'll gladly do it for you.
